Ryan joins a group of high school graduates filming an internet series investigating urban legends. However, he believes that this group is connected to the disappearance of his sister a few months ago. He thinks that one of the youngsters caused his sister's disappearance by playing the deadly Elevator Game and convinces them to play the game again. While playing the game, they awaken the Woman on the Fifth Floor and things take a terrifying turn.
